Hepatology
The Department of Gastroenterology and Hepatology was set up at SPARSH Hospitals (Yeshwantpur and Infantry Road) in early 2019 with an aim to provide comprehensive and affordable Gastro and Hepatology care to the surrounding population.
Gastroenterology and Hepatology is a rapidly changing field with technological advances allowing complex procedures to be done in a minimally invasive manner with no or minimal period of hospitalization. Double Balloon Enteroscopy:
The Double Balloon Enteroscopy has allowed visualization of deeper parts of the small intestine and to intervene / treat illness and lesions which earlier needed surgeries.
Intestinal ulcers can be biopsied, strictures dilated and bleeding lesions tackled with a DBE. SPARSH Hospitals is one of the prime centers performing Double Balloon Enteroscopy in Karnataka.
Capsule endoscopy: Capsule endoscopy is a small battery driven camera that is to be ingested and records the entire gastrointestinal tract to be visualized later on a computer. This is a painless way to visualize the entire gastrointestinal tract and help to achieve diagnosis / direct treatment in people with anemia / black stools of unknown origin, unclassified abdominal pain and malabsorption syndromes.
Endoscopic ultrasound: Endoscopic ultrasound helps us to visualize structures in and beyond the intestinal walls. It helps in accurate diagnosis and staging of gastrointestinal tumors,
CBD stones, pancreatitis as well as help us in decompressing – bile duct / pseudocyst and providing pain relief in pancreatitis via celiac plexus neurolysis.
Spy Cholangioscopy: Large sized bile duct stones were earlier subjected to surgical treatment. Nowadays moderate sized stones can be removed after large balloon sphincteroplasty where the papillary opening is dilated with a balloon and large stones can be removed after breaking them with a laser through a spy cholangioscopy.
Spy Cholangioscopy is a special type of endoscope which can be passed into the bile duct, visualize the stones and tumors within as well as break large bile duct stones with the help of laser. This has made management of complex bile strictures (narrowing) and large bile stones easy, safe and can avoid / help plan surgery better.
Manometry & pHmetry: Manometry helps to record muscle contractions and sequential
coordinated muscle activity involved in the act of swallowing and passing motion.
This aids in classifying disorders of swallowing as well as constipation so that directed therapy can be applied.
This is combined with 24-hour pHmetry – where a continuous record of the lower esophageal
pH measurements are taken in people with difficult acidity / GERD. These investigations help us select patients who would respond best to anti-reflux surgery.
Third Space Endoscopy: Third space endoscopy is the newest kid on the block of endoscopy and allows us to remove lesions which were considered as surgical indication. It is used to treat tumors within the wall of the gastrointestinal tract as well as a motility disorder called achalasia cardia.
Large polyps can also be removed endoscopically without the fear of bleeding of perforation (hole) by applying clips or a loop to the base of the polyps. A wide variety of foreign bodies are routinely and safely removed at SPARSH with the help of special hoods and baskets. Bleeding ulcers and varices are routinely managed with clips, bands and thermal coagulation. Argon plasma photocoagulation is applied to control bleeding from abnormal blood vessels and after radiotherapy. Obstruction because of tumors and narrowings are routinely managed with balloon dilation and stents as appropriate.

Surgical Gastroentorology

- Diverticular Disease: Diverticular disease occurs due to the aging of muscles that make up the wall of the large intestine. An infection occurs in the tiny pockets of the large intestine. The pockets are called diverticula. These pockets bulge out through weak spots in your large intestine. The pockets can become inflamed or infected.
- Inflammatory Bowel: Ailments that cause prolonged swelling of part or all of the digestive tract. Symptoms associated with inflammatory bowel are diarrhea, abdominal pain, blood in the stools, reduced appetite and unintended weight loss.
- Ulcerative Colitis: Ulcerative colitis is a common form of inflammatory bowel disease. The innermost lining of the large intestine becomes swollen, and ulcers may form on the surface.
